Faith Formation and Sacraments

Holy Name provides opportunities for all to deepen our knowledge and strengthen the foundation of our faith. We encourage all ages to participate in on-going formation. Faith Formation or Catechesis is the process of fulfilling Christ's mission to us to "go and make disciples." Catechesis comes from a Greek word meaning "to echo the teaching" and is a mission for all of us.

At Holy Name, Faith Formation includes:

Adult Faith Formation & OCIA

Preschool - 12 year olds

Sacraments of Initiation Preparation

Teen Ministry & Confirmation

  • Baptism

    Application

    Baptism of children 0 - 7 years old:

    • Official/Certified Copy of the Birth Certificate
    • Godparent (conditions apply)
    • Residency in Parish Boundaries or Membership in the Parish
    • Preparation program within the last 3 years
    • Strong consideration of participating in adult formation classes for both parents and godparents

    Baptism of children over 7 years old:

    • All of the above
    • Participation in OCIC with parents
    • Participation in family formation/teen ministry

    Baptism of adults:

    • Godparent (conditions apply)
    • Residency in Parish Boundaries or Membership in the Parish
    • Paricipation in OCIA, Adult Formation, and the GIFT program
    • Family participation in family formation is strongly encouraged

  • Eucharist

    • Must be baptized in the Catholic faith (if not see baptism requirements)
    • Must participate in the family formation and Preparing to Receive Jesus programs.
    • Must be in the Parish boundaries or registered members of the Parish
    • Parents are strongly encouraged to participate in adult formation classes.

  • Confirmation

    Starting fall of 2026, Confirmation will be a two year program. Those who register in 2026 will be confirmed Spring of 2028.

    Requirements:

    • Candidate must be baptized in the Catholic faith (if not see baptism)
    • Candidate must complete two-year confirmation prep program.
