Faith Formation and Sacraments
Holy Name provides opportunities for all to deepen our knowledge and strengthen the foundation of our faith. We encourage all ages to participate in on-going formation. Faith Formation or Catechesis is the process of fulfilling Christ's mission to us to "go and make disciples." Catechesis comes from a Greek word meaning "to echo the teaching" and is a mission for all of us.
“The name of catechesis was given to the whole of the efforts within the Church to make disciples, to help people to believe that Jesus is the Son of God...and to educate and instruct them in this life and thus build up the Body of Christ.” (Catechesi Tradendae #1)
At Holy Name, Faith Formation includes:
Adult Faith Formation & OCIA
Preschool - 12 year olds
Sacraments of Initiation Preparation
Teen Ministry & Confirmation

Baptism
Baptism of children 0 - 7 years old:
- Official/Certified Copy of the Birth Certificate
- Godparent (conditions apply)
- Residency in Parish Boundaries or Membership in the Parish
- Preparation program within the last 3 years
- Strong consideration of participating in adult formation classes for both parents and godparents
Baptism of children over 7 years old:
- All of the above
- Participation in OCIC with parents
- Participation in family formation/teen ministry
Baptism of adults:
- Godparent (conditions apply)
- Residency in Parish Boundaries or Membership in the Parish
- Paricipation in OCIA, Adult Formation, and the GIFT program
- Family participation in family formation is strongly encouraged
Eucharist
- Must be baptized in the Catholic faith (if not see baptism requirements)
- Must participate in the family formation and Preparing to Receive Jesus programs.
- Must be in the Parish boundaries or registered members of the Parish
- Parents are strongly encouraged to participate in adult formation classes.
Confirmation
Starting fall of 2026, Confirmation will be a two year program. Those who register in 2026 will be confirmed Spring of 2028.
Requirements:
- Candidate must be baptized in the Catholic faith (if not see baptism)
- Candidate must complete two-year confirmation prep program.
